Our annual flu vaccine clinic has been scheduled for Friday, October 25th, 2019 here at SJLES.  Paper consents will go home at the end of the week, or Monday, September 16th at the latest. Online consents are available again this year, and can be done now.  Please visit:

 Remember that the consent form will ask for insurance information, however, insurance is not required in order to receive this vaccine.  Please fill out the consent form (online or paper) for each student completely, and sign.If the signature is left blank, we cannot give your student the vaccine.
